- Please get your blank physical form from one of your coaches or print if off here: https://www.comalisd.org/apps/pages/StudentPhysical

- Physical forms and waivers must be signed and returned to your coach/athletic trainer PRIOR to the date of the physical so that they may be reviewed. DEADLINE - May 1, 2023.

- These are standard screening physicals for any major musculoskeletal and general medical issues for athletes in current grades 7-11. Current 6th grade athletes (rising 7th grade) should see their primary care physician as their first sports physical should be more than just a screening.

- Cardiac testing will be completed for incoming 9th-12th graders courtesy of August Heart https://www.augustheart.org/

- Physicals will be performed by Pieper Team Physicians - Sports Medicine Associates of San Antonio https://www.sportsmedsa.com/

- Must have an updated physical on file with the Athletic Trainer or coach before you are able to participate in any athletic practices for the 2023-2024 school year (new physicals must be dated after May 1 ).

- Please complete your online paperwork for the 2023-2024 school year at www.rankonesport.com

- Written Requests for refunds must be received prior to to midnight May 10, 2023.

- Refunds will not be issued for no shows and the amount will be accepted as a donation to the Pieper Sports Medicine program.

- WALKUPS will be $45 per physical, Cash/Checks are not accepted.
